It is a familiar narrative—healthcare providers meet patients during an admission, and it becomes increasingly clear that this may be their final hospital admission. As members of the palliative care team, we have the privilege to journey with patients and their families, as they confront what are likely surprising and drastic changes in their life (functionally, medically, emotionally, and spiritually). These are troubled times for patients and their loved ones. In this presentation, we share a patient’s journey through our institution. By facilitating and empowering his wish through a multidisciplinary effort, we share how he was able to find comfort amidst the ineluctable and peace among the chaos.
